Overview:
Chart review for proper diagnosis coding form various chart types (physician, hospital
outpatient, hospital inpatient). Insure that the highest level of diagnosis code was utilized in each date of service reviewed and documentation of findings in company data storage program.
Responsibilities:
May have occasional interaction with physician offices or insurance carriers to facilitate clarification and/or training on coding results.
May have occasional spec! ial projects that will entail a full coding audit.
Professionally communicates finds, errors, and any suggestions to all staff in order to facilitate on-going communications and efficient department operations.
Completes all responsibilities as outlined on annual Performance Plan.
Completes any special projects and other duties as assigned in a timely manner.
Qualifications:
High School Diploma; CPC, CPC-H, CCA, CCS or CCS-P (Nationally
certified medical coder as certified by either AAPC or AHIMA)
Preferred 5+ years of medical coding experience, including supervisory
experience.
Clinical background preferred, but not required.
Demonstrated high level of quality in clinical coding work.
Adherence to official coding guidelines, coding clinic determinations, CMS and other
regulatory compliance guidelines and mandates.
Expertise in medical coding, particularly diagnosis coding.
Excellent written and verbal skil! ls to include coaching and interpersonal skills.
Strong knowledge o! f medical terminology and anatomy and physiology.
Skills in organization and customer service.
Ability to read and understand medical record documentation.
Good computer and technological skills.
Must be able to work in a fast paced environment.
Analytical and problem solving skills.
Ability to manage and meet deadlines.
Requires sitting for long periods of time, some bending, stooping and stretching.
Good manual dexterity and eye-hand coordination is needed for use of computer, keyboard, photocopier, fax, and reference textbooks.
Work is performed in an office setting with some possible travel to physician
offices to gather needed data.
Must remain flexible to provide assistance in any emergent situations and/or
projects.
Must participate in any required training.
Must be able to perform duties with or without reasonable accommodation.
Must abide by all HIPAA and associated patient confidentiality requirements. Must be able to identify trends in coding and documentation errors, to include over
and under-coding.
Must maintain dignity and respect for all patients, family members, staff, visitors,
