Provides screening, counseling, case management, training or other specific t! reatment or rehabilitation activities as directed; does related work as required. Essential Duties
Provide case management services to include linking, monitoring and coordinating. Use knowledge of Medicaid Waiver and SPO to ensure appropriate provision and documentation of services. Follow Licensure and Medicaid regulations. Performs related work as required.
Knowledge of principles, practices and methods of treatment and/or rehabilitation of individuals with intellectual disabilities or mental illness; some knowledge of mental health and/or intellectual disabilities and related problems; good interpersonal skills which include the ability to empathize, demonstrate respect, communicate, reflect feelings and confront constructively; ability to relate freely to and work effectively with clients and professionals from all levels of the community; adherence to basic ethics appropriate for any helping relationship; ability to write concisely and coherently; demonst! rated ability to use good judgment. Minimum Qualifications
Possession of valid driver's license issued by the State of Virginia required. TB Screening and Child Protective Services Registry Check required, if offered employment.
Desire experience in the provision of Waiver Case Management and SPO Case Management. Prefer at least 1 year experience working with individuals with intellectual disabilities.
